- Student-Athlete must submit the following documents before acceptance to JAA.
- High school transcripts (from all high schools attended)
- College transcripts (if applicable)
- ACT/SAT test scores
- NCAA ID

- Register with the NCAA Eligibility Center. You must also meet the NCAA core coursework and testing requirements and satisfy the following conditions:
- Graduate from high school on time (eight consecutive semesters from the start of grade 9)
- Earn 10 of the 16 core courses BEFORE the 7th semester (senior year) of high school
- Earn seven of the 10 courses in English, Math, and Science
- Earn a minimum GPA of 2.0 in core courses on a 4.0 scale
- Retake any courses before the 7th semester. After the 7th semester, none of the first 10 core courses can be replaced.
- Earn a combined SAT score that corresponds with a grade point average for Division I schools.

- Enroll for Fall classes - DATE: Upon acceptance into JAA’s program. If you have not graduated yet, turn in current High School Transcripts (and College Transcripts, if applicable) and email a copy of a valid ID and your unofficial transcripts to the college.
- Attend Student-Athlete Orientation Summer Session at Achieve Fitness Center .

- Student-Athlete submit Proof of Physical to JAA.
- - DATE: Prior to first day of practice
- In order for you, the student-athlete, to compete in intercollegiate athletics here at JAA all of the necessary paperwork must be completed online each year. Student-athlete, a physical must be completed on paper to remain with the athletic trainer. You must use the Physical Form provided by JAA. Failure to use the JAA Physical Form will result in your physical being VOID. If ALL forms are not fully complete, there will be a delay in your participation in any intercollegiate practices and/or games here at JAA.
- All physicals for this academic year must be completed after June 1st. All physicals must be signed by a Medical Doctor or Doctor of Orthopedics. Physicals completed by any other health care provider must be signed by their supervising physician. Physicals performed by a chiropractor, not signed by a physician or on a form other than the JAA Physical form will not be accepted. A new physical will be required at the cost of the student-athlete. Download JAA’s Physical Form on the Compliance and Eligibility Page.

- JAA requires that all student-athletes show proof of insurance (i.e., under a parent, employer, etc...) prior to participating in an intercollegiate athletic program.
